Non NHS Examinations/Forms

If you need a medical examination for such things as life insurance, HGV license, pre employment check etc these can be arranged by appointment and will be subject to a fee.  The fee must be paid prior to examination.  Please see the receptionists for more information.

If you need any forms filling in and signing by the doctor then there will be a charge.  Please ask the reception staff for more information.  Payment is required before the completion of the form please and can take up to 30 working days for the GP to complete.
